Our Services

Physical Therapy: Contact us for pricing on evaluations and subsequent visits. Packages of 4 or 8 sessions are available at a discounted rate. An additional fee of $30 may apply if you are outside our treatment area. Cash rates are only available for non-Medicare patients.

Insurance: We are now in-network with Medicare and Tricare for physical therapy. All other patients will be cash pay. However, we provide receipts containing all the information insurances require for submission to cover out-of-network physical therapy services. Many clients have submitted our receipts and received reimbursement.

Dry Needling: $45 per visit for cash pay and Medicare patients.

Wellness Visits: $100 for an individual 50-minute session. $150 for a couple (2-person) 50-minute session.

We also treat rotator cuff injuries, ACL/PCL/MCL/LCL tears and repairs, hamstring and quadriceps injuries, Achilles tendon issues, plantar fasciitis, ankle sprains, piriformis syndrome, thoracic outlet syndrome, and many more conditions.

Do I need a doctor referral to see you?
No, in the majority of cases you do not need a doctor referral. Indiana is a direct access state, meaning physical therapists can see a patient without a doctor referral for the first 42 calendar days. Some insurances, including Medicare, do require a physician to be following your plan of care.

What forms of payment do you accept?
We accept cash, check, credit cards (Visa, Mastercard, American Express, Discover, Diners Club, and more), HSA, Apple Pay, Venmo, Zelle, Samsung Pay, and Google Pay.

Do you offer in-person clinic appointments?
Yes, we have expanded to include operations out of Vitalix Health & Fitness, located at 1117 South Rangeline Rd, Carmel, IN.

Why are your costs so low compared to therapy clinics?
We have very low overhead. By traveling to see you, we don't worry about the costs of owning or renting a building and we don't need any accessory staff or employees.
